Notes

The first and second editions of the book were printed copies.  They are no longer in print and are not available except in a few libraries around the country.  The first edition was printed in 1995, and the second edition was printed in 1996.

The third edition is an updated version of the second edition.  It was released on the Internet in 2006.  The third edition was never issued as a printed book, and we never plan to issue it in that form.  There are two reasons for this: 1) it is simply too expensive to print a book, and 2) once a book is printed, it is very difficult to make changes and updates.

There is no charge to read this book online.  If you like, you have my permission to print out a copy for your personal use.  Please keep in mind, however, that I revise and update this online version from time to time.  If you print out a copy, you'll miss all the changes.

Libraries have my permission to print out one copy (or several copies) of the book to put in circulation.  Again, please keep in mind that the book changes from time to time.  Once you've printed a copy, you won't see the latest changes.
